Spinal injury forces Phillies’ No. 1 prospect to undergo surgery

Shortstop Aidan Miller, the Philadelphia Phillies’ No. 1 prospect, will undergo surgery June 5 as a result of a recurrence of low back pain during his rehabilitation.

“Phillies infielder Aidan Miller has been diagnosed with discogenic pain and facet inflammation in the lumbar spine,” the team announced June 3. “Due to recurrence of low back pain when returning to baseball activities, he was seen by spine specialists over the last couple of weeks. The specialists were in agreement that the recommended course of treatment is a minimally invasive procedure called a radiofrequency ablation of the facet joints.

“Miller will have the procedure performed this Friday, June 5. He will rest for one week after the procedure and then return to his rehabilitation with the goal to return to game activity in six to eight weeks.”

Miller Is a consensus Top-30 prospect in baseball and the top minor league prospect in the Phillies’ organization. Last year he slashed .264/.392/.433 in 116 games at Double-A and Triple-A.

Although it’s a blow to Miller, the announcement clarifies why the 21-year-old has yet to make his 2026 debut with one of the Phillies’ minor league affiliates.

Optimistically, Miller is a core piece of the Phillies’ future as they look to transition from an older club that finished first, second or third in the National League East every season this decade, making the postseason in each of the last four.

Pessimistically, Miller is blocked at shortstop by 32-year-old Trea Turner and would be among president of baseball operations Dave Dombrowski’s best trade chips as the Aug. 3 deadline approaches.
